Support for Families Living with Alzheimer’s Disease and Other Forms of Dementia

Caring for someone with Alzheimer’s disease or another form of dementia can be overwhelming, especially when families are trying to manage appointments, safety concerns, behavioral changes, medications, personal care, and caregiver burnout at the same time. Many families in Northern Virginia are doing their best to keep a loved one safe and comfortable at home, but they may not know what support is available or how to coordinate the many pieces of dementia care.

The Medicare GUIDE program, formally known as Guiding an Improved Dementia Experience, was created to help eligible Medicare beneficiaries with dementia and their unpaid caregivers receive more coordinated support. For families in Fairfax, Arlington, Alexandria, McLean, Reston, Woodbridge, Burke, Ashburn, Centreville, Springfield, Oakton, and throughout Northern Virginia, GUIDE may be an important resource to explore.

What Is the Medicare GUIDE Program?

The Medicare GUIDE program is a dementia care model from the Centers for Medicare & Medicaid Services. Its purpose is to improve quality of life for people living with dementia, reduce stress on unpaid family caregivers, and help more individuals remain safely in their homes and communities.

GUIDE focuses on coordinated dementia care. Instead of leaving families to figure everything out on their own, GUIDE connects eligible individuals and caregivers with a care team that can help assess needs, create a care plan, provide education, coordinate services, and offer ongoing guidance as dementia progresses.

For many families, this type of support can make dementia care feel less fragmented and more manageable.

Who May Be Eligible for GUIDE?

Eligibility is determined through a participating GUIDE provider and Medicare. In general, a person may be eligible if they:

• Have a dementia diagnosis confirmed by a clinician
• Are enrolled in Original Medicare Parts A and B
• Have Medicare as their primary payer
• Live at home or in an approved community setting
• Are not enrolled in Medicare Advantage, PACE, or Medicare hospice
• Are not already aligned with another GUIDE participant
• Families should speak with a participating GUIDE provider or Medicare representative to confirm eligibility.

Why Family Caregivers Need Support

Dementia caregiving can be physically and emotionally exhausting. Family members may find themselves providing care around the clock, managing repeated questions, nighttime confusion, wandering concerns, personality changes, or resistance to personal care.

Many caregivers delay asking for help because they feel responsible for handling everything themselves. Over time, this can lead to stress, fatigue, missed work, poor sleep, and health concerns for the caregiver.

Programs like GUIDE recognize that dementia care must include the caregiver. When family caregivers receive education, guidance, respite, and practical help at home, they are often better able to continue supporting their loved one with confidence and compassion.
