Phone Consultation

Living Room Visit
We’ll visit you and your parent in your home or discharge facility to dig into your situation and story. We’ll answer any question you have, meet your parent and start to build a relationship and gain a shared understanding of the level of home care you need and desire.

START OF CARE
We’ll develop an initial care plan and match you with a trained caregiver to start delivering in-home care. After 30 days we’ll review the services and satisfaction and evolve the care plan to better fit any other needs that exist.

Post-Discharge: Ensuring Clients Are Safe at Home
Have concerns about a family member returning home after inpatient care? Contact us so we can help your loved one and allay your fears.
- One Hour On-Site Response: When you contact us, we come to the hospital, facility, doctor’s office, or home within one hour to meet the patient’s family. On site, we will discuss our services, our options, and our costs.
- Teamwork With Your Clinicians: IncrediCare’s registered nurses (RNs), licensed practical nurses (LPNs), and certified nursing assistants (CNAs) interact with your facility’s occupational and physical therapists and nursing staff on site, familiarizing themselves with the patient’s therapy requirements, confirming all patients’ needs and making sure instructions are followed.
- Close Management of Other Therapies: Our staff ensures the patient’s consistent wellness and treatment progress.
- Plan of Care in Place at Discharge: “Welcome Home” – we’re ready! Introduction of the RN, LPN, and home aide upon start of services. We provide ongoing RN supervision.
- For 25 years, IncrediCare has been known for our responsive, 24-hour on-call service across Northern Virginia. We are now serving Arlington, Alexandria, Fairfax, Loudoun, and Prince William counties. The process of scheduling care is not difficult. Contact us, answer some simple questions, and we will respond quickly to arrange the care you need in less than 2 hours. Our care managers will be there to meet with your family at the location you specify within 60 minutes.
